Urgent appointments

  • use an online form to request an appointment – You can request either a telephone appointment or a face to face appointment.
  • If you are an elderly patient or do not have online access, please call us on 02085485960 and a member of our team can complete the form for you. You can also attend the Practice where a member of our team can assist you with the form.

When you get in touch, we’ll ask what you need help with.

However you choose to contact us, we may offer you a consultation:

  • by phone
  • face to face at the surgery
  • on a video call
  • by text or email

Automated Check-in

You can check yourself in for your appointment without having to queue at reception. Look for our ‘arrival touch screen’ which is located on the wall near to the Main entrance. Please make sure you check using the machine or with one of the receptionists at the front desk to avoid missing your appointment.

If you need help when we are closed

In an emergency or life threatening situation dial 999 and ask for an ambulance.

For urgent medical advice when we are closed, please ring NHS 111 or you can call the surgery number 0208 548 5960 to be connected to the Out of Hours GP

If you need help with your appointment

Please tell us:

  • if there’s a specific doctor, nurse or other health professional you would prefer to respond
  • if you would prefer to consult with the doctor or nurse by phone, face-to-face, by video call or by text or email
  • if you need an interpreter
  • if you have any other access or communication needs

Home visits

We encourage patients to come to our accessible surgery however, we appreciate this is not always possible.

If you are housebound or are too ill to visit the practice you can request a home visit. Your GP will only visit you at home if they think that your medical condition requires it.

We will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls.

If you do need a home visit, you can help us by calling reception before 10:00am
