Sick / Fit notes

You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days.

The NHS website provides further information on when a sick note is required.

Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.

To request a sick note or renew one

If you are unable to work and are ill for longer than seven days or you need to renew a sick note you can complete an online form. We’ll respond within 3 working days.

If you have not had a consultation with a clinician, you will need an appointment before they can issue it.  If you’ve already had an appointment with a clinician, out of hours or hospital about your illness, we can send your request to the clinician in the practice.

Please be aware that sick notes are not urgent and it can be backdated for you. You can inform your employer that it is in progress and if they require further information, you can give the surgery number for them to contact us. We can send it to you electronically to your mobile number if you are unable to come to the surgery to collect it.
